Data We Collect
Cardholders Contact Data- Individual Cardholder contact data including State (ID Card or Drivers License) or Federally issued ID (Green Card, Passport, Resident Alien Card) number.
- Cardholders Guardian Contact Information (If minor or a ward of a facility/state or another individual)
- Cardholder ICE (In Case of Emergency) Contacts (Primary and Secondary)
- POA (Power of Attorney) Contact (POA for medical decisions)
- Patient Primary Care Physicians Office (Primary Contact)
- Clergy/Priest/Rabbi/or other Spiritual Counselor and cardholders religious preference.
- Biometric Profile of the eMedyx Cardholder – Two forefinger (or first available readable digit on each hand) biometric print compatible with HSPD12 / FIPS201 standards. This biometric profile is to be captured by the primary physician or issuing organization at the time of the first encryption of the card.
The Biometric Profile shall be used solely for the eMedyx System and shall be incorporated within the eMedyx System in a double-blind system such that the biometric portion of the data is completely separate from the rest of the eMedyx System and ONLY returns the eMedyx ID Number for the cardholder when the biometric system is used in IDENTIFICATION mode.
It is the stated intent of eMedyx and all its' partners within the eMedyx Network that the biometric information of the cardholder is NOT to be shared with any entity outside of the eMedyx System for cardholder verification of identity or identification in case of incapacity without the eMedyx Card being present. The biometric profile or any other information deemed non-medical shall not be shared outside of the eMedyx Provider network (including disclosure to State or Federal law enforcement or other non-medical agencies without a direct court order by a Federal Court of competent jurisdiction). No officer, board member, director, manager, or other employee of Data Processing Services, LLC or any of their partners has the right or authority to override this directive and shall if found in breach be prosecuted to the fullest extent of the law.
Cardholders Current Medical Conditions
- Conditions - Including ID9 / ID0 (2008 Current Codes)
- Physician/Specialist making the diagnosis with NPI Code(s)
- Date of Diagnosis
- Current Treatment Regimen
Cardholders Current Medications / Allergies / Immunization Record with date of last physical.
- List of Medications (with unique FDA/NIH code) including Brand Name & Generic choice with latest prescribing physician or specialists NPI Code and the current dispensing pharmacy if available with NPI code.
- List of over the counter (OTC) medications currently being used by cardholder
- List of herbal or other dietary supplements currently being used by cardholder
- Allergies to medications, foodstuffs, or other environmental allergens
- Cardholders Current Immunization record
- Immunization record shall include the date of last complete physical.
